Requirements for application for an Unabridged Birth Certificate

¡P      - Form: BI-154 (It is available at office)

¡P      - Passport and ID

¡P     - Full names, SA ID numbers, place and country of birth of both Parents of the applicant

¡P      - Fee: NT$140.

 

Note: If the applicant does not have an ID Number then follow the process for registration of birth.
