To apply, you must submit the following documentation:

1738: duly completed (fill out form p.80 ~ 86 and p.96) ¡V available
at the office

colored photographs (measuring 34 x 45mm, from head top to chin between
29mm~34mm.) Against white background, showing ears, mouth: relaxed. Teeth
should not be showing. The photo should not be older than 3 months.
¡P Valid
passport which expires in no less than 30 days after expiry of the intended
date of departure from the Republic.

yellow fever vaccination certificate
¡P A
medical report.
¡P A
radiological report
¡P Alien
Residence Certificate (ARC): for the passport holder who is other than
Taiwanese.
¡P Marriage
certificate or in the case of a foreign spousal relationship, proof
¡P of
official recognition thereof issued by the authorities of the foreign
¡P country
of the applicant (where applicable).
¡P Divorce
decree, where applicable.
¡P Death
certificate, in respect of late spouse, where applicable.
¡P A
police clearance certificate shall only be required from the police or
security authorities in each country where the applicant, after having
attained the age of 18 years, has resided for 12 months or longer during the
five years immediately preceding the date of application
¡P Non-refundable
fee in cash.
¡P The
minimum monthly payment to a foreigner from a pension fund or an irrevocable
retirement annuity or a net worth or a combination of assets realising the
amount determined from time to time by the Minister by notice in the
